Interventional Cardiologists Take Another Stand Against Maintenance of Certification

The Society for Cardiovascular Angiography & Interventions (SCAI) is urging the American Board of Internal Medicine (ABIM) to eliminate procedural volume reporting requirements for maintenance of certification (MOC). The group asserts that the current thresholds, including 100 percutaneous coronary intervention (PCI) or equivalent procedures over 2 years or an alternative low-volume pathway, are burdensome, lack evidence as reliable measures of competency, and may have unintended consequences such as reduced patient access to emergent PCI in rural areas. SCAI contends that procedural volume does not accurately reflect physician quality or outcomes and is inconsistent with modern competency-based frameworks. The proposal has received support from the American College of Cardiology, which similarly maintains that competency should be assessed through continuous learning and performance rather than numerical procedure counts. SCAI further argues that interventional cardiology is uniquely subject to these requirements among ABIM-certified specialties and opposes replacing volume thresholds with other reporting metrics. SCAI's draft statement is open for public comment until September 18, 2026.
