At Stony Brook University Renaissance School of Medicine in New York, the accredited continuing medical education (CME) team, led by Dorothy S. Lane, MD, MPH, associate dean for continuing medical education, partnered with the Stony Brook Diabetes Center and Hospital Quality Improvement Department to improve inpatient diabetes care. By bringing clinicians from multiple departments together around a shared patient safety goal, the initiative aligned electronic ordering, glucose monitoring, and point-of-care practices with the American Diabetes Association's Standards of Care and leveraged a diabetes tracking dashboard to identify and manage at-risk patients. As a result, severe hypoglycemia events among hospitalized patients with diabetes fell by 35% (from 2.10% to 1.36%), HbA1c testing rates increased from 85% to 97.1%, and 30-day readmission rates declined from 14.2% to 13.6%. The findings demonstrate how accredited CME, interdisciplinary collaboration, and quality improvement strategies can drive better patient outcomes.
