Gaming in Medical Education: Promise, Pitfalls and Practice

Gaming and gamification are increasingly being incorporated throughout medical education, from pre-clerkship training and clinical education to residency and continuing professional development. Tools such as quiz platforms, virtual reality, simulations, escape rooms, serious games, and mobile applications are used to enhance engagement, reinforce learning, and provide safe opportunities to practice clinical decision-making. Evidence suggests that, when paired with deliberate practice, feedback, and structured debriefing, these approaches can improve learner motivation, short-term knowledge retention, procedural skills, and collaboration. However, challenges include the risk of superficial learning, overreliance on rewards, unequal access to technology, and concerns that competition may undermine professionalism or trivialize patient care. As a result, successful implementation requires thoughtful instructional design that emphasizes competency-based learning, psychological safety, inclusivity, and reflection, positioning gaming as a complement to rather than a replacement for traditional educational methods.
